Semple, Grant, & Benario Awards Application Form

Please try to include many of your materials as possible with this on-line application, as you will be able to submit it only once. If it is necessary for you submit some materials separately, please do so via email to camws@camws.org. Keep in mind, however, that award recipients must have complete files.

The individuals you list as recommenders will also receive emails informing them of your request. Once a letter of recommendation is submitted on your behalf, you will be notified via email. As the deadline approaches, please do not hesitate to remind your recommenders if you do not receive such confirmation.

Please note that files must be complete (including all letters of recommendation) for candidates to receive an award. The subcommittee may read and rank incomplete files, but no incomplete file may receive the award.
